7K a year Monticello House is a skilled nursing facility located in Jackson, Mo.
We are currently accepting applications for our MDS Coordinator position.
The ideal candidate will be a Registered Nurse, but will accept a Licensed Practical Nurse preferably with prior experience as a MDS Coordinator.
RN/LPN license must be in good standing with the State Board of Nursing.
Knowledge of skilled nursing facilities survey process and regulations would be helpful.
The MDS Coordinator is responsible for coordinating the completion of the resident assessment in accordance with current Federal and State regulations.
Assists with the development of a comprehensive resident assessment and care plan.
Participates in resident/family care plan meetings.
Reviews and validates reports and ensures appropriate follow-up action is taken.
Participates in various meetings and committees:
Medicare, QAPI, Weight Management etc.
